Murabbi Development Program at ICNA Center Waiver:
I am the parent/legal guardian of the student, or I am personally the student referred to in this document. I acknowledge that participation in activities at ICNA Center, including classes held by ICNA, involves a risk of injury, accidents, and other health issues. By signing this form, I fully waive any legal claims against ICNA, including its employees, contractors, volunteers, and board members, for any injuries, losses, or damages suffered by myself, my child, or our property.
Emergency Medical Authorization:
I permit ICNA and its affiliates to obtain emergency medical treatment for the student if I or the emergency contacts are unavailable.
Risk Assumption for Physical Activities:
As a parent/guardian, I understand that any physical activities the student engages in at ICNA Center are done at their own risk. This includes but is not limited to the use of exercise equipment, sports facilities, and other premises of ICNA Center. I acknowledge the risk of injuries or damages that may result from these activities, including those caused by the possible negligence of the staff or trainers at ICNA Center.
Release of Liability:
I agree to release ICNA and all associated personnel from any claims related to injuries or damages that may arise from the student's participation in activities, use of equipment, or presence on ICNA Center premises, including those resulting from potential negligence.
Photo Release:
During the program, we take photographs of activities involving students to share the program's positive experiences and updates. By which incidentally, some pictures may capture your child's participation, directly or indirectly.
These photos may be published through our website, newsletters, and social media pages. With this, we seek your consent in allowing us to publish photos which may involve your child to the said platforms. By proceeding, you allow the reproduction and publication of your child's photograph(s).
Indemnification:
I agree to hold ICNA harmless against any costs, losses, or damages they incur related to the student’s activities or presence at ICNA Center facilities.
Acknowledgment of Understanding:
I confirm that this waiver and release is comprehensive and is legally binding. If part of this agreement is deemed invalid, the remaining parts will remain effective. This document specifically excludes claims of gross negligence or intentional harm. I have carefully read and understood this document and accept its terms voluntarily, acknowledging it forms a legally binding agreement. No additional oral agreements or inducements have been made related to this waiver and release.
